I am about to get married. My foreskin does not come back completely.
What can happen if I do not do circumcision? Is there alternative to circumcision?

WHAT ARE THE ADVANTAGES AND DISADVANTAGES OF CIRCUMCISION?
There are web sites dedicated detailing the disadvantages of circumcision and there are, interestingly, sites detailing the advantages of circumcision.
Practically for sexual and reproductive health, as well as general health circumcision has neither any advantage nor disadvantage.
WHAT MAY HAPPEN IF CIRCUMCISION IS NOT DONE?
In a man who's foreskin cannot be pulled back completely to expose the glans penis or the tip part of the penis there rarely may be

[2] BLEEDING
[3] PARAPHIMOSIS

PAIN
Pain is experienced occasionally during vigorous sexual intercourse as the prepuce gets forcibly stretched behind.
This pain is mild to moderate. It may prevent one from doing immediately another sexual intercourse. The pain rapidly decreases in a day. Sometimes the pain, which is always mild and bearable takes few more days to vanish and sexual activity can begin.

Fortunately this type of pain does not occur all the time. It occurs rarely in the entire full sexual life time of a man. Most people never experience such pain.
1 CIRCUMCISION
If the pain occurs every time, then alone, one may consider surgical removal of the entire fore skin.

2 KEEP PRACTICING 'PULLING BEHIND'
Frequently practice pulling the foreskin behind so that it stretches and over time it can come back much more than it did initially. You may do this in the bathroom or in the place of privacy like the bedroom. You may use (soap in the bathroom) hair oil while practicing 'PULLING BEHIND'.
IMPORTANT INSTRUCTION
Remember to place the foreskin as it usually is, that is covering the entire tip portion of penis.
If during sexual intercourse the guy experiences pain it is most of the times due incomplete retraction of penis in erect condition.

BLEEDING
The frenulum may tear a bit. This could cause bleeding noticed during sexual intercourse. Most often bleeding often without the pain. It is not serious though it can cause the couple to panic if they do not know these medical facts.
Occasionally in ones life time even without the difficulty of retraction of prepuce bleeding occurs because of vigorous sexual intercourse due to abrasion.
This is transitory and almost often stops by itself.

Bleeding is very scary for the person/couple.
Just as you would treat any bleeding from a cut or scratch or wound you
Press the affected part for about 3 minutes. You may have to repeat this until bleeding totally stops.

Clean the genitals with water. Make sure you do not pull open to check the abrasion. This will interrupt body's natural clot mechanism, which has stopped the bleeding. If you go about checking the cut portion (affected area) oozing recurs. Follow procedure FIRST.

(a)What medication to apply?
It is not advisable to apply ointment/medications/oil/medicated powder.
(b) What to wear?
Just wear a tight underwear so that you prevent friction on the penis.
You may use a light loose clothing like a lungi so that even underwear does not rub against it. Take this care for a day or two.
(c) Dressing
Use clean cotton cloth to tie around the penis. It will absorb the slight bleeding and help prevent friction with clothing or body parts. You may try using sanitary napkin. This is hardly ever necessary, unless you panic and do not want to disclose to your doctor or do not have a doctor/family physician at that time to guide you.

If left undisturbed it may take up to 4 or 6 hours for the oozing of blood to stop. Natural clot that covers the broken skin, protects it and helps in total healing does the work.
It should not stop you from resuming your office work once you are sure the bleeding is stopped.

If the blood keeps of gushing out and does not stop by the above means (RARE),
you are advised to visit a Urosurgeon. After examination, the Urosurgeon may apply antiseptic dressing or choose to do circumcision.

PARAPHIMOSIS
When the foreskin, which is difficult to be pulled back has comeback REMEMBER to push it forwards in its original place, that is covering the glans or the tip part of the penis. Thus let the foreskin be placed in its original position covering the entire penis.
Sometimes it may cause PARAPHIMOSIS.
Paraphimosis is a painful.
The foreskin acts as a constricting band. The glans gets swollen. You are not able to manually retract the prepuce. This is known as paraphimosis.

CIRCUMCISION or retracting the prepuce under anesthesia.
HOW DO IN KNOW IF I NEED TREATMENT OR CIRCUMCISION WHEN MY FORESKIN DOES NOT COME BACK FULLY?
To find if there is phimosis and if you need treatment is it advisable to get examined by a doctor. If necessary circumcision is done. Mere inability to draw back the prepuce during erection does not require surgical removal of foreskin. For phimosis, typically circumcision is required in childhood. In phimosis when there is inability even in passing urine and the foreskin opens minimally, circumcision is becomes necessary and is done in childhood.
